An Architecture Decision Record (ADR) captures one material decision: the context, the options your team weighed, the tradeoffs, the decision, and who owns the follow-through. It is the document that lets a decision survive beyond the meeting where it was made.

How is the governance maturity score calculated?
After the record is written, a short set of questions checks how your team handles decisions today: where records live, who reviews them, and how often they are revisited. The result is a maturity level with the gaps named.
Where does an ADR fit in a larger review?
ADR candidates are a named output of the Azure Architecture Assessment, alongside the risk register and the 90-Day Action Plan. Teams that already write ADRs use the assessment to confirm the decisions behind them before more work is approved.
